Why Aren't We Seeing Faster AI Adoption in Healthcare?

We build software for a living. We work with APIs, data, and automation every day. We have seen first-hand how quickly technology can move from concept to production when the conditions are right. So it is genuinely frustrating to watch an industry where AI could save lives continue to drag its feet.

Healthcare is one of the most obvious candidates for AI-assisted decision making. Pattern recognition in medical imaging, early detection of disease markers, triaging patients based on symptom data - these are problems that machine learning is genuinely good at solving. The technology exists. In many cases, it has been proven to work. And yet adoption remains painfully slow.

This is not a technology problem. It is an everything-else problem.

The Regulatory Maze

Healthcare is one of the most heavily regulated industries in the world, and for good reason. When a decision can mean the difference between life and death, you cannot afford to get it wrong. But the regulatory frameworks we have were designed for a world of physical devices and pharmaceutical trials, not self-learning algorithms that evolve over time.

In the EU, the AI Act entered into force in August 2024 and classifies most healthcare AI systems as "high-risk," which triggers a significant set of compliance obligations. The first obligations around general-purpose AI models kicked in during early 2025, with the full requirements for high-risk systems not fully applicable until 2027. That is a three-year runway just to get the rules in place, let alone to build, certify, and deploy compliant systems.

In the United States, the FDA has authorised over 690 AI and machine learning-enabled medical devices, but the approval process remains complex and time-consuming. Each device needs to demonstrate safety and efficacy, which is entirely reasonable - but the traditional approval model does not account for algorithms that learn and change after deployment. The FDA is developing lifecycle-based oversight models, but these are still evolving.

The result is a regulatory environment where innovation moves at the speed of technology but approval moves at the speed of bureaucracy. Developers are building tools that could be deployed today, but the legal framework to support them will not be ready for another two years.

Who Is Liable When AI Gets It Wrong?

This is arguably the single biggest barrier to adoption, and it does not have a clear answer yet.

Under current malpractice law in most jurisdictions, the physician remains responsible for clinical decisions regardless of whether AI was involved. If a doctor follows an AI-generated recommendation that turns out to be wrong, it is the doctor who faces the malpractice claim - not the algorithm, not the developer, and not the hospital that chose to deploy it.

This creates an impossible situation. Physicians are being asked to use tools they cannot fully understand or verify, while bearing the entire legal burden when those tools fail. It is like asking a pilot to fly a plane with an experimental autopilot system, but holding only the pilot responsible if it crashes.

The liability question involves multiple parties: the developer who built the algorithm, the hospital that procured and deployed it, the clinician who acted on its recommendation, and potentially the regulatory body that approved it. But existing legal frameworks do not distribute responsibility across these parties in any meaningful way.

The EU considered introducing an AI Liability Directive that would have applied non-fault rules to high-risk AI failures, but this was withdrawn in early 2025. Without clearer liability frameworks, healthcare organisations are understandably cautious about deploying systems that could expose them to legal risk without any corresponding legal protection.

The Trust Deficit

Trust is a recurring theme in every study on AI adoption in healthcare. Clinicians do not trust AI systems they cannot understand. Patients do not trust diagnoses made by machines. Hospital administrators do not trust technology that has not been validated over years of clinical use.

This lack of trust is not irrational. Healthcare professionals have spent years - sometimes decades - developing clinical intuition. Asking them to defer to an algorithm that cannot explain its reasoning is a significant cultural shift. It is not enough for AI to be accurate. It needs to be explainable, auditable, and transparent in how it reaches its conclusions.

The problem is that many of the most powerful AI models are inherently opaque. Deep learning systems that deliver the best diagnostic accuracy are often the hardest to explain. There is a fundamental tension between performance and interpretability, and healthcare is one domain where you cannot sacrifice either.

Building trust takes time, and it requires consistent evidence of reliability. A single high-profile failure can set adoption back by years. The pressure to get it right the first time makes organisations risk-averse, which in turn slows down the very deployment that would build the evidence base needed to establish trust.

The Black Box Problem

This deserves its own section because it sits at the heart of so many other barriers.

When an AI system analyses a medical image and flags a potential tumour, clinicians need to understand why. Was it the shape? The density? The location relative to other structures? Without this information, the recommendation is essentially "trust me" - and that is not how medicine works.

The lack of explainability creates problems at every level. Clinicians cannot verify the reasoning, so they cannot catch errors. Regulators cannot audit the decision-making process, so they cannot approve the system with confidence. Patients cannot understand why a particular diagnosis was reached, so they cannot give truly informed consent. And when something goes wrong, nobody can determine whether the fault lay in the algorithm, the training data, or the clinical context.

Research into explainable AI (XAI) is progressing, but it has not yet reached the point where complex diagnostic models can provide the kind of clear, human-readable explanations that healthcare demands. Until it does, the black box problem will continue to be a significant barrier.

Data Quality and Bias

AI is only as good as the data it is trained on, and healthcare data is notoriously messy.

Electronic health records are inconsistent across providers and systems. Medical imaging standards vary between equipment manufacturers. Historical data often reflects existing biases in healthcare delivery - if certain demographics have historically been underdiagnosed, an AI trained on that data will perpetuate the same gaps.

Data quality issues are compounded by privacy regulations. Health data is among the most sensitive personal information there is, and accessing it for AI training requires navigating a complex web of consent requirements, anonymisation standards, and data protection laws. The EU's European Health Data Space is attempting to create a framework for secondary use of health data, but it only entered into force in 2025 and will take years to fully implement.

The result is that AI developers often work with datasets that are too small, too narrow, or too biased to produce models that generalise well across diverse patient populations. A diagnostic tool trained primarily on data from one demographic group may perform poorly - or even dangerously - when applied to another.

The Cost Question

Healthcare organisations operate under enormous financial pressure, and AI implementation is not cheap. Beyond the cost of the technology itself, there are expenses related to infrastructure upgrades, staff training, workflow redesign, ongoing monitoring, and regulatory compliance.

For smaller hospitals and clinics, these costs can be prohibitive. Even for larger health systems, the return on investment is often unclear. Studies suggest that health systems with proper AI governance achieve ROI faster, but establishing that governance structure is itself a significant investment.

The financial case for AI in healthcare is compelling in theory - reduced diagnostic errors, faster treatment pathways, fewer unnecessary procedures - but proving it in practice requires the kind of large-scale deployment that the other barriers on this list are preventing. It is a chicken-and-egg problem.

What Needs to Change

None of these barriers are insurmountable, but addressing them requires coordinated effort across multiple stakeholders.

Regulators need to develop frameworks that are specific to AI in healthcare, rather than trying to fit adaptive algorithms into approval processes designed for static medical devices. The EU AI Act is a step in the right direction, but its healthcare-specific guidance is still being developed, and the timeline for full implementation extends to 2027.

Liability frameworks need to distribute responsibility fairly across developers, deployers, and clinicians. Holding physicians solely responsible for decisions influenced by systems they cannot fully understand is neither fair nor sustainable.

AI developers need to prioritise explainability alongside accuracy. A model that delivers a 2% improvement in diagnostic accuracy but cannot explain its reasoning is less useful in clinical practice than a slightly less accurate model that can show its working.

Healthcare organisations need to invest in governance structures, training programmes, and change management. Technology adoption is as much a cultural challenge as a technical one, and treating it purely as an IT project is a recipe for failure.

And the industry as a whole needs to accept that building trust takes time. Rushing deployment to capture market advantage without adequate validation and governance will only reinforce the scepticism that is already holding adoption back.

We are not healthcare specialists. We build APIs and web applications. But we understand what it takes to move technology from prototype to production, and we see the same patterns in healthcare AI adoption that we see in other industries struggling with digital transformation.

The technology is rarely the problem. The problems are organisational, regulatory, cultural, and legal. They are the unglamorous, slow-moving challenges that do not make for exciting product announcements but determine whether a technology actually gets used.

AI has the potential to transform healthcare in ways that genuinely save lives. But potential is not enough. Until the surrounding infrastructure - legal, regulatory, organisational, and cultural - catches up with the technology, adoption will remain frustratingly slow.

And every month of delay is a month where diagnoses that could have been caught earlier were missed.
